از واکسیناسیون دانشآموزان تا محدودیتهای هوشمند در تمام کشور؛
کدام واکسن برای کودکان بهتر است؟
پوشش واکسیناسیون برای همه گروههای سنی درحال کامل شدن است از جمله دانشآموزان، با وجود این دبیر کمیته علمی ستاد کرونا معتقد است برای بازگشایی مدارس نباید عجله کرد چراکه ممکن است سبب پیک جدید شود، همچنین با کامل شدن واکسیناسیون محدودیتها هوشمند میشود طوریکه اینبار افرادی که واکسن تزریق نکردند با برخی محدودیتها مواجه خواهند شد.
با سرعت گرفتن عملیات واکسیناسیون علیه ویروس کرونا بخش زیادی از افراد جامعه حداقل دُز اول واکسن خود را دریافت کردند. طوریکه سامانه ثبت نام وزارت بهداشت محدودیت سنی برای دریافت واکسن را لغو کرد و از همه گروههای بالای ۱۸ سال تقاضا کرد که با ورود به این سامانه مقدمات تزریق واکسن را انجام دهند.
اما این پایان کار نبود، بعد از آن وزیر بهداشت اعلام کرد با پوشش کامل واکسیناسیون افراد بالای ۱۸ سال قرار است واکسیناسیون برای گروه سنی زیر ۱۸ سال در دستور کار قرار بگیرد. اقدامی که سببساز اما و اگرهای زیادی شد، بهویژه درخصوص نوع واکسنی که قرار است برای این گروه سنی استفاده شود. گروهی از خانوادهها نگران عوارض واکسن برای فرزندان خود شدند و در این میان برخی از متخصصان هم گفتند مطالعات روی واکسنهایی که در سبد واکسیناسیون ایران وجود دارد برای تزریق به گروه سنی زیر ۱۸ سال کامل نیست و نیاز به تحقیقات بیشتری دارد.
چرا واکسیناسیون کودکان ضرورت دارد؟
به گزارش روزنامه «صبح امروز» به نقل از خبرآنلاین، با این وجود از قرار معلوم واکسیناسیونِ این گروه سنی شروع شده و متولدین سالهای ۱۳۸۵ به بالا میتوانند برای دریافت واکسن در سامانه واکسیناسیون ثبت نام کنند. اما همزمان با اعلام این خبر آنچه دارد پرسیده میشود ضرورت واکسیناسیون این گروه سنی است، چراکه مطالعه رفتاری کرونا نشان داده این ویروس کمترین بیماریزایی و مرگومیر را برای همین گروه سنی دارد.
در این باره دکتر حمیدرضا جماعتی، دبیر کمیته علمی ستاد کرونا میگوید: «درصد درگیری بچهها نسبت به افراد بزرگسال خیلی کمتر است، در مطالعاتی که انجام شده شاید تقریباً ۵ درصد بچهها به کووید درگیر شدند. همچنین بیماری بچهها خفیف است و درصدی از آنها که وارد بیمارستان و آیسییو میشوند نسبت به بزرگسالان بهشکل چشمگیری کمتر است.
منتهی نکتهای که وجود دارد این است که چرخش ویروس میتواند از طریق بچهها انجام شود، درنتیجه تاکید بر بسته شدن مدارس بهاین دلیل بود که ویروس از طریق بچهها میتوانست در خانوادهها چرخش پیدا کند و به افراد مسن مانند پدربزرگ و مادربزرگها منتقل شود. بنابراین افرادی که در ریسک بالا هستند بهدلیل درگیری به ویروس کارشان به بیمارستان و آیسییو و خدایی ناکرده به فوت میرسید. لذا یکی از عواملی که میتواند سبب کنترل پاندمی در جهان شود این است چرخش ویروس را در میان بچهها کنترل کنیم.»
وی با اشاره به حداقل و حداکثر سن درنظر گرفته شده برای واکسیناسیون کودکان و نوجوانان ادامه میدهد: «الان ۱۲ تا ۱۸ سالهها مدنظر ما هستند. البته در بعضی از کشورها واکسیناسیون این گروه سنی را برای زیر ۱۲ سالهها هم انجام میدهند. یعنی آنها از ۳ سال به بالا دارند همه افراد جامعه را واکسن میزنند که شامل حال ۱۲ تا ۱۸ سالهها هم میشود.»
اگر واکسیناسون این گروه سنی انجام شود، با توجه به انجام شدن واکسیناسیون بزرگسالان میتواند کمک کند که در چند ماه آینده ما بتوانیم مدارس را بازگشایی کنیم. اما اینکه بگوییم حتماً در آبان این اتفاق میافتد الان نمیشود قول داد
اما و اگرهای واکسن «سینوفارم» برای کودکان!
از روزی که گفته شد قرار است واکسیناسیون گروه سنی زیر ۱۸ سال در دستور کار قرار بگیرد پرسشهای زیادی درباره نوع واکسنی که باید به آنها تزریق شود، مطرح شد. بسیاری از کشورهای دنیا که تزریق واکسن به این گروه سنی را شروع کردند از واکسن فایزر، مدرنا و سینوفارم استفاده میکنند، حالا ایران قرار است در کنار چین، امارات و بحرین تبدیل به چهارمین کشوری شود که برای واکسیناسیون زیر ۱۸سال واکسن سینوفارم را انتخاب کرده است.
با این وجود آنطور که دبیر کمیته علمی ستاد کرونا میگوید محدودیتی در انتخاب واکسن وجود ندارد و میشود از انواع واکسنهای تایید شده استفاده کرد: «تمام واکسنهایی که بهوسیله وزارت بهداشت و سازمان غذا و دارو بهعنوان واکسن ضد کرونا تایید میشود برای این گروه سنی قابل استفاده است. بنابراین اهمیت ندارد که بگوییم حتماً باید این یا آن واکسن باشد. هر واکسنی که وزارت بهداشت تایید کند، میتواند مورد استفاده قرار بگیرد بهخاطر اینکه در بچهها دیده شده که واکسیناسیون عوارض جدی ندارد و معمولاً عوارض کمی دارد و تازه همین عوارض کم را هم در تعداد محدودی از بچهها از خودش نشان میدهد.»
جماعتی ادامه میدهد: «علائمی هم که واکسن برای کودکان دارد مانند یک سرما خوردگی یا تب و لرز خفیف، ضعف بدن یا کمی سر درد است که در یکی دو روز خوب میشود. بنابراین چون عوارض واکسنها کم است و کارایی بالا دارد چندان نیاز نیست که ما تاکید روی واکسنی خاصی داشته باشیم. اگرچه واکسن سینوفارم و واکسن پاستور که مشترک میان ایران و کوبا است جزو واکسنهایی است که برای این گروه سنی استفاده میشود. اما همانطورکه گفتم هر نوع واکسنی که سازمان غذا و دارو تایید کند مورد استفاده برای تزریق به بچهها قرار میگیرد.»
او در پاسخ به این پرسش که «اینکه گفته میشود واکسین سینوفارم در کشور خودمان و برای گروه سنی زیر ۱۸ سال آزمایش بالینیاش انجام نشده و همین مسئله کمی شک و شبهه برای استفاده از این واکسن ایجاد میکند صحیح است؟» میگوید: «خیر، در دنیا مطالعات زیادی راجع به آن درباره همین گروه سنی انجام شده است، عوارض بسیار ناچیزی برای بچهها دارد؛ بنابراین واکسن خیلی خوبی است و ما مشکلی با این واکسن نداریم. ضمن آنکه تزریق آن برای کودکان مانند بزرگسالان بهصورت دو دُز انجام میشود.»
واکسیناسیون کودکان، بازگشایی مدارس، شروع یک پیک جدید!
حالا که قرار است گروه سنی زیر ۱۸ سال واکسینه شوند زمزمههای بازگشایی مدارس قویتر بهگوش میرسد، اما آنچه اهمیت دارد عدم تعجیل در اینباره است. جماعتی ضمن اشاره به پیشبینی از زمان بازگشایی مدارس یادآور میشود نباید این مسئله شروع کننده پیک جدیدی از شیوع کرونا در کشور شود.
دبیر کمیته علمی ستاد کرونا میگوید: «پیشبینی در این باره به سرعت انجام واکسیناسیون بستگی دارد، ما با آقای دکتر عیناللهی وزیر بهداشت جلسهای داشتیم و ایشان در جلسه اعلام کردند نزدیک به ۵۰ درصد جمعیت واکسیناسیون شدهاند، این رکورد بسیار بالایی است که در این مدت کوتاهی که ایشان بودند توانستند میزان واکسیناسیون را از حدود ۸ تا ۱۰ درصد به حدود ۵۰ درصد برسانند. در مراکز متعدد، از مساجد گرفته تا بسیج، همچنین پارکها و ورزشگاهها مشغول تزریق هستند و چون نیروهای مردمی و بسیج وارد شدند و به انجام واکسیناسیون کمک کردند خوشبختانه سبب شد ما الان به درصد بالایی برسیم و این بسیار خوب است. درباره بچهها همین مسئله صدق میکند، اگر همکاری لازم انجام شود من فکر میکنم میشود این واکسیناسیون را طی یکی دو ماه آینده انجام داد.»
وی ادامه میدهد: «اگر واکسیناسون این گروه سنی انجام شود، با توجه به انجام شدن واکسیناسیون بزرگسالان میتواند کمک کند که در چند ماه آینده ما بتوانیم مدارس را بازگشایی کنیم. اما اینکه بگوییم حتماً در آبان این اتفاق میافتد الان نمیشود قول داد. چراکه همهچیز به سرعت واکسیناسیون و همکاری مردم بستگی دارد، هرچه سریعتر انجام شود این بازگشایی هم اتفاق میافتد. منتهی باید تاکید کنم نباید چندان عجله کرد، اگر قبل از واکسیناسیون کامل برای بازگشایی مدارس عجله کنیم ممکن است با پیکهای جدید مواجه شویم.»
ادامه محدودیت کرونایی برای کسانیکه واکسن تزریق نمیکنند
حالا که پوشش واکسیناسیون برای بزرگسالان حداقل در دُز اول انجام شده و تلاشها برای واکسیناسیون گروه سنی زیر ۱۸ سال ادامه دارد وزیر بهداشت صحبت از لغو برخی محدودیتها به میان آورده است، دبیر کمیته علمی ستاد کرونا با اشاره بههمین مسئله میگوید: «ما هیچ مطالعهای نداریم که نشان داده باشد قرنطینههایی که انجام دادیم در کنترل کرونا چقدر کارآیی و اثر بخشی داشتند. حتی آن محدودیتها سبب شد مسافرتهای بینشهری اتفاق بیفتد. بنابراین معلوم نیست این محدودیتها چه اثری داشتند. ضمن اینکه با انجام واکسیناسیون محدودیتها هوشمند میشود، یعنی کسانیکه واکسن کرونا تزریق کردند اجازه تردد شبانه داشته باشند سبب میشود محدودیتها تا حدودی زیادی از بین برود.»
او در پاسخ به این پرسش که پس بهدنبال ارائه خدمات به ازای واکسینه شدن هستید؟ ادامه میدهد: «در بسیاری از کشورهای دنیا همینطور است. ببینید ما غیر از موازین بهداشتی مانند استفاده از ماسک، شستن دستها با آب صابون و یا محلولها پایه الکل، همچنین انجام فاصلهگذاری اجتماعی، برای کنترل این پاندمی راهی جز واکسیناسون نداریم، بنابراین واکسیناسیون و رعایت نکات بهداشتی دو اقدام اساسی برای کنترل پاندمی هستند.
حالا اگر کسانی این مسئله را قبول نداشته باشند؛ چون آنها سبب اختلال در کنترل پاندمی میشوند ممکن است که یکسری محدودیت داشته باشند، مثلاً کسی میخواهد مسافرت برود نیاز به ارائه کارت واکسیناسیون است. اگر کارت واکسیناسیون نداشته باشند سبب منع رفتوآمد و انجام کارهایی میشود که ممکن است سبب انتقال عفونت در جامعه شود. بنابراین این راه حلی است که در آن، نه آن اجبار مدنظر وجود دارد و نه اینکه فرد رها شده است تا سبب ایجاد اشکال در کنترل پاندمی کرونا در جامعه شود.»
جماعتی، دبیر کمیته علمی ستاد کرونا در پایان یادآور میشود: «اعمال چنین محدودیتهایی زمانی است که ما واکسیناسیون را کامل انجام داده باشیم و دیگر کسی نباشد که بخواهیم به او واکسن بزنیم و اعلام شود فعلاً مراجعه به هیچکدام از مراکز واکسیناسیون نداریم. در اینصورت یک بازه زمانی تعریف میشود و آنجا اعلام میکنند کسانیکه واکسن نزدند به این چند مرکز مراجعه کنند، بعد از آنکه فردی برای تزریق واکسن مراجعه نکرد با توجه به عدم در اختیار داشتن کارت واکسن عواقب تصمیمی که گرفته است با خودش خواهد بود.»
