واقعیت گردشگری سلامت در ایران ؛
هماهنگی در سیاستگذاری و نابسامانی در اجرا

گردشگری سلامت به عنوان یکی از انواع گردشگری، قدمتی دیرینه در ادوارمختلف تاریخ بشر و جایگاهی ویژه از نظر تولید ثروت در حوزه اقتصاد گردشگری دارد. بر اساس تعریف سازمان جهانی گردشگری ( UNWTO )، گردشگری سلامت عبارت است از سفر به مکانی خارج از محل سکونت فرد جهت استفاده از خدماتی که به بهبود یا افزایش سلامتی و افزایش روحیه فرد ( با استفاده از آب‌های معدنی، آب‌وهوا یا مداخلات پزشکی) می انجامد.
برای بهبود وضعیت گردشگری سلامت در کشور چه باید کرد؟
آن چه در حوزه گردشگری سلامت به عنوان یک موضوع نخ نما در طول 15 ساله اخیر مطرح بوده و به مهم ترین چالش صنعت گردشگری سلامت تبدیل شده، عدم تعامل سازنده میان وزارت میراث فرهنگی، گردشگری و صنایع دستی و وزارت بهداشت، درمان و آموزش پزشکی است آنچه از ابتدا مورد مناقشه این دو دستگاه اجرایی متولی بوده است اصرار وزارت بهداشت در ورود به فعالیت هایی است که ذاتا خاصیت گردشگری دارد و دلیل سپردن آنها به شرکت های تسهیلگری که باید دغدغه مهم درمان داشته باشند خیلی مشخص نیست. اگر چه این مناقشات سابقه طولانی دارد پس از مدتها تفاهمنامه مشترکی بین مقامات عالی دو دستگاه اجرایی فوق به امضا رسید که طی آن مراکز درمانی دارای IPD ملزم به همکاری با شرکت های خدمات مسافرتی و گردشگری دارای مجوز از سازمان میراث فرهنگی ، گردشگری و صنایع دستی شده بودند ولی در عمل وزارت بهداشت با صدور شیوه نامه و صدور مجوزهایی به عنوان شرکت های تسهیلگر درمان از همکاری با شرکت های خدمات مسافرتی و گردشگری خودداری کرد که این امر تا کنون با اعتراض گسترده ای از سوی شرکت های خدمات مسافرتی و گردشگری دارای مجوز گردشگری سلامت از وزارت میراث فرهنگی، گردشگری و صنایع دستی مواجه شده است. بعدها این مناقشه به شورای راهبری گردشگری سلامت کشیده شد و بعد از طرح و تصویب آن در آن شورا برای تصویب نهایی به دبیرخانه هیات وزیران ارسال و در جلسه هیات وزیران مطرح و تعیین تکلیف گردید. چنان که در بند پ ردیف ۱۱ جدول احکام آخرین مصوبه هیات وزیران به شماره ۶۶۶۳۴/ت ۵۵۲۹۳ ه مورخ ۱۳۹۹/۰۶/۱۶ مبنی بر ابلاغ سند راهبردی گردشگری کشور صراحتا تاکید شده که وزارت بهداشت ، درمان و آموزش پزشکی برای ارائه خدمات تشخیص درمان و امور پزشکی به گردشگران سلامت بایستی با شرکت های خدمات مسافرتی و گردشگری دارای مجوز از وزارت میراث فرهنگی و گردشگری و صنایع دستی همکاری نماید و در همین مصوبه مقرر کرد چنانچه شرکت های تسهیلگر درمان متقاضی فعالیت در امور گردشگری هستند باید نسبت به اخذ مجوز بند ب (شرکت خدمات مسافرتی و گردشگری ) از وزارت میراث فرهنگی و گردشگری و صنایع دستی اقدام نمایند.
ولی در حال حاضر آنچه بر روی کاغذ نوشته شده با آنچه اجرا می شود مغایرت اساسی دارد و متاسفانه بیشتر دانشگاه‌های علوم پزشکی کشور و به خصوص دانشگاه علوم پزشکی مشهد از اجرای تصویبنامه هیات وزیران به بهانه عدم ابلاغ مصوبه فوق از سوی وزارت متبوع خود و همچنین اعلام این موضوع که سپردن بازار گردشگری سلامت به دست آژانس‌های مسافرتی بازگشت به گذشته یعنی دلالی و واسطه‌گری و ارائه خدمات بی‌کیفیت، بی‌شناسنامه و گران به بیماران بین‌المللی و در نتیجه خدشه دار شدن وجهه کشورمان در حوزه سلامت خواهد بود؛ با مکاتبه با بیمارستان‌های تحت پوشش دولتی و غیر دولتی مبنی بر عدم همکاری با شرکت های خدمات مسافرتی و گردشگری، رسما از اجرای مصوبه فوق جلوگیری کرده اند و البته هیچ توضیحی هم ارایه نکرده اند که چرا قبلا که تمشیت امور صرفا از سوی وزارت بهداشت انجام می شده، اوضاع به سامان نبوده و چگونه است که دلالان و واسطه‌های غیرمجاز با همکاری بیمارستان ها و مراکز درمانی در حوزه گردشگری سلامت جولان می دهند؟!!
حلقه مفقوده بین وزارت بهداشت و گردشگری
بدیهی است عدم اجرای مصوبه هیئت دولت به دلایل واهی و ناهمسویی وزارتخانه های متولی در حوزه اجرایی پذیرفتنی نیست و این موضوع باید یک بار برای همیشه تعیین تکلیف شود همان طوری که وزارت میراث فرهنگی، گردشگری و صنایع دستی، شرکت های خدمات مسافرتی و گردشگری فعال در حوزه سلامت را از هرگونه مداخله در امورتخصصی درمان منع کرده و آن ها را ملزم کرده در امور تخصصی بخش درمان از مشاوره و همکاری پزشکان بهره بگیرند لذا انتظار می رود وزارت بهداشت، درمان و آموزش پزشکی از هرگونه ورود به ارایه خدمات گردشگری خودداری نماید. هرچند به نظر می رسد برقراری ارتباطات و تعاملات بین وزارت میراث فرهنگی ، گردشگری و صنایع دستی و وزارت بهداشت ، درمان و آموزش پزشکی حلقه مفقوده ای است که باعث شده علی رغم گذشت سال های متمادی گردشگران سلامت همچنان دچار سردرگمی و بلاتکلیفی باشند ودلالان و مترجمان بدون مجوز عملا هدایتگر بخش عمده ای از بازار گردشگری سلامت کشور باشند در صورتی که اگر هر کدام از این دو وزارتخانه به وظایف تخصصی خودشان بپردازند و به عبارتی دیگر وزارت میراث فرهنگی، گردشگری و صنایع دستی و شرکت های خدمات مسافرتی و گردشگری فعال در حوزه سلامت متعهد به انجام امور گردشگری شده و وزارت بهداشت ، درمان و آموزش پزشکی نیز صرفا در چارچوب بیمارستان ها به ارائه خدمات درمانی بیماران پرداخته و هرکدام در انجام موارد غیر تخصصی از امکانات و تخصص طرف های مقابل استفاده کنند .
آنچه مسلم است با توجه به پیش بینی ورود سالانه ۲۰ میلیون گردشگر خارجی طی سال های آتی به کشور و ضرورت استفاده از ظرفیت های گردشگری سلامت جهت جذب ۲ تا ۲.۵ میلیون گردشگر بین المللی سلامت به نظر می رسد ساز و کارهای موجود به هیچ عنوان متناسب با اهداف تعیین شده نیست و صرفا با تجمیع و هم افزایی امکانات و پتانسیل های موجود، باید زمینه جذب حداکثری گردشگران سلامت و خدمات رسانی به بیماران خارجی را فراهم کنیم.
در این راستا ضرورت دارد شورای راهبری گردشگری سلامت کشور با برگزاری جلسات مستمر با حضور نمایندگان دستگاه‌های متولی در این حوزه شامل ورارت میراث‌فرهنگی، گردشگری و صنایع‌دستی، وزارت درمان، بهداشت و آموزش پزشکی، وزرات امور خارجه، دستگاه های اجرایی امنیتی و انتظامی، اتاق بازرگانی ایران و سازمان نظام پزشکی با تبیین جایگاه ها و وظایف تخصصی هر یک از دستگاه های اجرایی و اتخاذ تصمیمات گروهی برای ایجاد تغییر بنیادی در وضعیت موجود، دستیابی به سیاست جذب حداکثری گردشگران سلامت و ارائه خدمات درمانی با کیفیت از طریق مجاری قانونی به آنها بیشتر تلاش کنند.
